Family Registry (Koseki)

Filing records associated with the Family Registry

The family registry (koseki) is a system managed by the government that records the births, deaths, and marital/parental relationships of Japanese citizens. Main applications associated to the family registry are shown below. These applications can be completed at the Citizen Affairs Division (Shimin Seikatsuka) in Kamogawa City Hall, the Amatsu-Kominato Branch Office, or other city branch offices.

Birth (Shussei-todoke)

  • Who Must File:
  1. Father or mother
  2. Legal proxy
  3. Roommate
  4. Doctor or midwife who witnessed the birth
  5. Other witness
  6. Head of public facility
  • When:Within 14 days after birth

 If the 14th day is a holiday, the deadline will be the following business day.

  • Where to File:The local government office of one of the following:
  1. Permanent home address (honsekichi)
  2. Place of birth (shusseichi)
  3. Current address (jushochi)
  4. Notificant's current residence (shozaichi)
  • What to Bring:
  1. Birth certificate
  2. Maternity health handbook
  3. Personal seal (inkan), or if you do not own one, a valid signature is also acceptable

Death (Shibo-todoke)

  • Who Must File:
  1. Relative
  2. Roommate
  3. House owner
  4. Land owner
  5. Caretaker of house
  6. Caretaker of land
  7. Head of public facility
  • When:Within 7 days after known time of death
  • Where to File:The local government office of one of the following:
  1. Deceased person's permanent home address
  2. Place of death (shibochi)
  3. Notificant's current address or residence
  • What to Bring:
  1. Certificate of death OR postmortem certificate
  2. Personal seal (inkan), or if you do not own one, a valid signature is also acceptable
  3. Cremation fee (13,000yen)
  4. Photo ID (alien registration card, driver's license, etc.)

Marriage (Kon'in-todoke)

  • Who Must File:Husband and wife
  • When:Within 3 months if marriage was established overseas
  • Where to File:The local government office of the husband and wife's permanent residence, current address, or current residence
  • What to Bring:
  1. Marriage notification form
  2. For foreign nationals, proof that the marriage is legal in their own country
  3. Copy of complete family register
  4. Photo ID (driver's license, passport, etc.)

Divorce (Rikon-todoke)

  • Who Must File:Husband and wife
  • When:Within 10 days after the final decision, in case of adjudicated (arbitrated) divorce
  • What to Bring:
  1. Divorce notification form
  2. Copy of family register (only if the permanent home addres is not in Kamogawa)
  3. Copy of arbitrated record or tribunal report and transcription if the divorce was settled in court
  4. Photo ID (alien registration card, driver's license, etc.)

 

 

  • When submitting a form regarding the family register, you must present a government-issued photo ID (driver’s license, passport, etc.) that can be used to confirm your identity. If you do not have any of the above documentation, please present both your health insurance card and your pension book.
  • Depending on the type of notification you are submitting, you may be required to complete health insurance and/or pension procedures.
