Certificates needed for identification

At the Citizen Affairs Division, you can request various identification records needed to process official uses. The table below lists the most major types of certifications. When requesting a certificate, please bring the necessary documents and processing fee to the Citizen Affairs Division, located on the 1st floor of the City Hall.

Additionally, when requesting a certificate, you must present a government issued photo ID (driver’s license, passport, etc.) that can be used to confirm your identity. If you do not have any of the such documentation, please present both your health insurance card and your pension book.

Address Certification (for kei car).Jusho Shomeisho (keijidoshayo)

  • What to Bring:Personal seal (inkan), or if you do not own one, a valid signature is also accepted.

Copy of Complete Family Register.Zenbu Jiko Shomeisho (Koseki Tohon)

  • Fee per Copy:450yen
  • What to Bring:
  1. Personal seal (inkan), or if you do not own one, a valid signagutre is also accepted.
  2. Photo ID (Driver's license, passport, etc.)

Individual Registration Certificate.Kojin Jiko Shomeisho (Koseki Shohon)

  • Fee per Copy:450yen
  • What to Bring:
  1. Personal seal (inkan), or if you do not own one, a valid signagutre is also accepted.
  2. Photo ID (Driver's license, passport, etc.)

Certification of Family Registration Acceptance.Koseki no Juri Shomeisho

  • Fee per Copy:350yen
  • What to Bring:
  1. Personal seal (inkan), or if you do not own one, a valid signagutre is also accepted.
  2. Photo ID (Driver's license, passport, etc.)

Certified Copy of Foreign Registration.Gaikokujin Toroku Genpyo Kisai Jiko Shomeisho

  • Fee per Copy:350yen
  • What to Bring:
  1. Personal seal (inkan), or if you do not own one, a valid signagutre is also accepted.
  2. Photo ID (Driver's license, passport, etc.)

Certificate of Personal Seal Registration.Inkan Toroku Shomeisho

  • Fee per Copy:350yen
  • What to Bring:Personal Seal Registration Booklet (Inkan Torokusho)

How to request a copy of family registration records via mail:

The City Hall accepts requests for copies of family registration via mail for residents who cannot come because of work or other engagements.

Please enclose the following information, a postal money order (for the processing fee), and a self-addressed and stamped return envelope into a single envelope and send it to the City Hall, Citizen Affairs Division:

  1. Name, address, and phone of the person applying.
  2. Name of the person who is the head of the family register.
  3. The names and addresses of the persons whose records you are requesting.
  4. How many copies of the Copy of Complete Family Register and(or) Individual Registration Certificate you are requesting.
  5. A copy of a photo ID (alien registration card, driver's license card) of the person applying.
