2 もらう日と 時間を 決めます。
市民生活課（04-7093-7855）に 電話してください。
電話できない人は、市役所に 来て、予約します。
もらえる時間
月、水～金 9時00分～16時30分
火 9時00分～18時30分
※土・日・祝日は、やっていません。
3 市役所1階 市民生活課に 行きます。
暗証番号（パスワード）が 2つ 必要です。自分で 決めます。
もってくるもの
- 交付通知書
- 通知カード
- 在留カード または 特別永住者証明書
（運転免許証、パスポートでも いいです。）

You will need to make a reservation
How to get your card:
Fill out the postcard you receive with the correct information
Make an appointment for the date and time you will go to pick up your card
Call the city hall to make an appointment: Tel 04-7093-7855
Available hours:
Monday, Wednesday - Friday 9:00 - 16:30
Tuesday 9:00 - 18:30
※ Closed on weekends and holidays.
Come to the first floor of the city hall, and go to the Citizen Affairs Division
You will have to make two different passwords (or verification numbers) for your My Number card.
Your main password has to be 6 - 16 characters long and contain numbers and alphabetic characters.
You will use this password when signing in online to pay or file taxes, among other uses.
Your secondary password will be 4 numbers.
You will need this password when using convenience store services like getting a Certificate of Residence, or Stamp Registration.
When you use certain online shopping services (through the Individual Number Card Point system) you may also need to use these passwords.
What to bring:
- Your filled out notification postcard
- Your Notification Card (social security number card)
- Your Foreign Residence card or Confirmation of Special Permanent Residence
（Driver's License, Passport)
