Registering a Positive COVID-19 Test / 陽性者登録〈検査で 《新型コロナウイルス》に かかったことが わかりました〉（COVID-19）
検査キットや PCR検査を 病院じゃないところ〈家など〉で した人は、かかったことを 「新型コロナウイルス感染症検査キット配布・陽性者登録センター」に 登録します。
お金は いりません。次の ぜんぶに あてはまる人は、登録してください。
- 千葉県に 住んでいます。
- 49歳以下です。
- 次の 病気を 持っていません。
- 性閉塞性肺疾患
- 慢性腎臓病
- 糖尿病
- 高血圧
- 心血管疾患
- BMIが 30以上じゃ ありません。太っていません。
- 妊娠していません。 ＜おなかの 中に 子どもが いません。＞
登録するとき、次の 書類の 写真を 送ります。
- 身分証明書（運転免許、マイナンバーカード、在留カード）
- 検査の 結果
- ワクチンの 注射を 打ったことが わかる書類

If you tested positive for COVID-19 using a kit or another location other than a hospital, you will have to register your case information with the “COVID-19 Test Kit Distribution and Patient Information Center”.
It doesn’t cost any money to do so.
If you are any of the following:
- Under 50 years old
- You don’t have any underlying conditions
- lung disease
- kidney disease
- high blood pressure
- cardiovascular conditions …etc.)
- You are not overweight (BMI under 30)
- You are not pregnant
…you will have to register.
You will also have to upload pictures of the following documents:
- Personal ID (driver’s license, foreign residence card, My Number Card, etc.)
- Your test results
- COVID-19 Vaccination receipt/coupon
