病院に　行く前に　ボールペンで　ぜんぶ　書いてください。
ワクチンについての　書類を　しっかり　読んでから　書いてください。

予防接種を　する日

子どもの　体の　具合が　いいか　調べます。
注射を　する日に　具合が　悪いとき、医者と　相談してください。

病院に　持っていくもの

  • 書いた予診票
  • 子どもの　健康保険証
  • 母子手帳

次の　とき、子どもは　注射できません。

  • 37.5℃以上　熱が　あります。
  • 重い　急性疾患に　かかっています。
  • 予防接種の　成分で　アナフィラキシーが　起きました。
  • 免疫機能に　異常が　ある病気が　あります。免疫抑制を　治しています。
  • 医者が　注射できないと　言いました。
注射の　後に　気をつけます
  • 注射を　したら、30分は　病院に　います。急に　副反応が　起きるかもしれないからです。医者と　連絡が　とれるように　します。
  • 注射を　して　1週間は、副反応が　起きないか　気をつけます。
  • 注射を　したところは、きれいに　します。風呂に　入っても　いいです。注射したところは、こすりません。
  • 注射を　した日は、運動しません。
  • 注射を　したところが　おかしかったり、体の　具合が　悪くなったり　したら、すぐに　医者に　診てもらいます。
予防接種健康被害救済制度

注射を　した後、 治らない病気や　障害に　なったとき、助ける制度です。 予防接種法で　決まっています 。
病院などで　かかったお金が　戻ってきたり、 障害に　なったとき、 年金が　もらえます。
申し込みや　相談は、市役所で　できます。

Day of vaccination

Check if your child is feeling well.
If your child is not feeling well on the day of vaccination, please consult with your doctor.

When going to the hospital, please bring…

  • Health and Vaccination History Form (Already filled out)
  • Child’s Health Insurance Card
  • Mother and Child Booklet

If your child is under the following conditions, child can not be vaccinated:

  • If child has a fever of over 37.5 degrees
  • If child has a serious acute illness
  • If anaphalyxis will occur due to the contents of the vaccine
  • If your child has an anomaly with immune function and if child is curing immunosuppression.
  • If doctor advises to not have the child vaccinated.
What to do after receiving the vaccine
  • After vaccinations, please stay at the hospital for 30 minutes. In case of sudden side effects, the doctor will be able to tend to you.
  • 1 week after being vaccinated, watch out for side effects.
  • Make sure that the vaccinated are is cleanand it is okay to take a bath. Please do not rub the area.
  • Do not exercise on the day you got vaccinated.
  • Please consult your doctor immediately when the vaccinated area is making you feel uncomfortable and if you don't feel well.
About the Relief System for Injury to Health with Vaccination

Vaccinations can cause health problems (illness or disability). Although this is extremely rare, the risk cannot be eliminated, and a relief system has been established for this reason.
In the case of health damage as a result of the vaccine from the vaccine, relief (medical expenses, disability pension benefits, etc.) is also available under the Immunization Act. Please consult with the municipality where your residence is located regarding the procedures required for application.
