ワクチンについて、書いてある紙を　しっかり　読んでから　注射を　してください。

予約が　いります。

手紙に　入っている病院の　リストを　見てください。
病院に　電話で　予約します。

予診票を　書きます。

病院に　行く前に　予診票を　書いてください。

Before getting vaccinations

You will need to make a reservation with the hospital before getting a vaccination for your child. Please check and call the hospital you want to go.

Health and Vaccination History Form (Yoshinhyo)

You MUST fill out a Health and Vaccination History Form before going to the hospital for these vaccinations. Please check the example below for how to fill out the form:
